Added the VoiceDisableMic debug setting to completely disable the ability to open the mic.
Also hoisted PTT key handling from LLVivoxVoiceClient up to LLVoiceClient. This cleans up LLVoiceModuleInterface a bit and makes the PTT logic more centralized. Reviewed by Richard.
